Q. What is PlanPlus?
A.
PlanPlus is a discount membership program that offers discount prescription plans and savings on other health services such as vision, including laser eye surgery (sometimes called LASIK eye surgery); hearing, with digital hearing aids and discount hearing aids; podiatric and chiropractic care. PlanPlus also has a discount dental plan that provides discounts on things like teeth whitening and provides a great dentist directory for finding a dentist in your area. All the PlanPlus discounts are available from a nationwide network of over 104,000 providers. PlanPlus even has a 24-hour Ask a Nurse hotline.

Q. How do I get my PlanPlus discounts?
A.
About two weeks after joining, you will receive a membership kit containing your PlanPlus membership card, a list of providers in your area and a detailed description of the discounts available. Simply show your card at participating provider locations and your discount will be applied at the time you receive the products or services.

Q. How do I confirm provider participation or make an appointment?
A.
We want you to save the most money with your PlanPlus membership. When you call your participating provider to schedule an appointment, we encourage you to mention the Network Name that supports the provider group. For your reference, below is a list of PlanPlus services and their corresponding network. Please note, your particular product configuration may not include all services - please review your fulfillment inserts and your membership card for the services/networks included in your plan.

Q. What are the PlanPlus dental discounts?
A.
Many dental care procedures are discounted. You will receive discounts of 10-50% off the national average price for preventive and diagnostic care and a discount of 10% off the network providers' regular fees for all other procedures.

Q. Are all drugs covered?
A.
All FDA approved prescription drugs are covered.

Q. Who can use this plan?
A.
Immediate family members in your household, including spouses, children under 18 and children over 18 who are attending school, and financially-dependent relatives.

Q. What are the PlanPlus vision discounts?
A.
Discounts off eye examinations and eyeglasses are available from the Coast to Coast network. Participating providers, such as Sears and LensCrafters, offer discounts from 10% to 30% off eye exams, 20% to 60% off regular retail prices of frames and lenses, and 10% to 30% off LASIK procedures.

Q. What are the PlanPlus hearing discounts?
A.
Discounts of 10% to 20% are offered through a network of audiologists throughout the United States.

Q. What are the PlanPlus podiatric discounts?
A.
Discounts from 20% to 50% are offered through podiatric specialists.

Q. What are the PlanPlus chiropractic discounts?
A.
Discounts from 30% to 50% are offered through participating providers.

Q. Who is included in my household?
A.
Members of your immediate family who live at your address are eligible to use the PlanPlus membership. The membership also covers any dependents of the member living away from home. However, only the member's name appears on the membership card.

Q. What if I'm traveling or moving and need to use one of the services?
A.
Call our toll-free Customer Service line at 1-800-309-3800 and we will be glad to supply you with provider names in your area. Or, access our provider search listing on the membership website provided in your membership kit.

Q. Are the prices for each service the same at all dentists or do they vary from dentist to dentist?
A.
For all preventive dentistry there is a fixed fee that every listed provider has agreed to use. You will receive this schedule with your kit. The fees reflected in the schedule will be subject to change from time to time. For all other procedures not included in the fees schedule, there is a discount of 10% off the network dentist's regular fees.

Q. Can I find out discounted prices from the dentist in advance?
A.
Yes, you may contact the dentist in the network, identify yourself as a PlanPlus member and request a price for a specific procedure.

Q. Do I have to wait until I get the PlanPlus card or can I get the discounts now?
A.
You will have to wait until you receive your membership card before obtaining the discounts. The provider will need certain information from the card.

Q. If I lose my list of providers, can you send me a new one?
A.
We are always happy to send you a new list. The provider listings are located online or through our toll-free Customer Service line at 1-800-639-9819. Provider listings are mailed first-class and generally take 5-7 days to deliver.

Q. Can I fill a one-year supply of prescription medicine using the pharmacy mail-order service?
A.
No, the maximum prescription quantity filled at one time is a 90-day supply.

Q. What if a PlanPlus provider won't accept my card?
A.
Each provider in our system has a contract with their provider network. When speaking with the provider, please mention the Network Name that supports the provider group. For your reference, below is a list of PlanPlus services and their corresponding network. Please note, your particular product configuration may not include all services - please review your fulfillment inserts and your membership card for the services/networks included in your plan.

Q. What if I need a new card/kit?
A.
We are always happy to issue another card/membership kit. Just call our toll-free Customer Service line at 1-800-309-3800. You will receive your card/membership kit within 7 to 10 days. The card will be in the name of the member.

Q. Can I get an additional card for my spouse or child?
A.
Any member of your household may use your card. If you need an additional card, we will be happy to supply one for you. You will receive it within 7-10 days. It will have the member's name on it as well.

Q. What if I move and need to change my address?
A.
Just call our toll-free Customer Service line at 1-800-309-3800. We will send a provider list for your new location.
