equipment, and you are obligated to pay for all medical services, pharmaceutical supplies, prescription drugs, or medical
equipment provided. You will receive discounts for medical services at certain providers who have contracted with the plan.
The range of discounts for medical ancillary services may vary depending upon the service rendered, provider used and geographic
location. A list of plan providers including the name, city, state, and specialty is made available prior to purchase at
www.planplus-healthcare-jcp.com.
You have a right to cancel your membership within 30 days of the effective date for a full refund of fees paid. You are not obligated to make any further payments or access the plan for the period of time after the last month for which payment has been made.
